Applying for disability can be a grueling process. Knowing what is involved will help you understand and tolerate the systems limitations. When you apply online, by phone or in person you will be sent a disability starter kit. This kit contains useful information regarding the application process and it provides forms to document pertinent information. It will help the process to go smother and more rapidly if you take the time to gather all the requested information before your interview.

Adult Disability Report

You can usually fill out this form online. If you have trouble with the online form then you can fill out the form on the phone or in person. Most of the information in the report will be requested during your interview but if you have done the report it is easier to give the answers since they are at your fingertips. This form asks basic personal information questions like name, address, phone number and Social Security Number. You will also be asked about your disability and how it affects you. Keeping a diary of symptoms and problems caused by your disability is invaluable. You will need to fill out a medical history. You will need to list the Doctors seen and the dates that you saw them. You will need to list medications, prescription and non-prescription, and why you are taking them. You will also need to list all the tests you have had or are scheduled to have and the name and address of where the test was performed. Next you will need to complete your work and education history. You will need to list all jobs that you have had in the last 15 years. You will also need to fill out language preference along with work difficulties related to your disability. Also, you need to know the last day you worked or if you are working now how many hours you are working etc.

Interview

Even if you have filled out the online disability report you must keep your interview appointment. You may ask to be interviewed by phone or you may choose to have your interview in person. Have all the forms from your kit and the disability report with you during your interview. You will also need all the paperwork requested on the checklist, including things such as birth certificate.

Medical Records

To be able to process your application Social Security needs the names, addresses and dates of all medical visits. Medical visits includes tests, hospitalizations, ER visits and Doctors office visits. If you have seen anyone for any type of therapy you need to also list that. The more complete and accurate your information is, the better chance you have of a favorable outcome. You will not need to get your medical records, they will be requested by the State office.

Paper work chain

Once your disability report has been received it will be reviewed for completeness. You will be contacted regarding any information that is unclear or missing. If you have not completed your formal application than Social Security will contact you to complete it. Once that is done the forms will all go to the State’s eligibility determination office. And then your medical records will all be requested and reviewed.
