According to the Public Debt Bureau of the US government, lost, stolen, mutilated, or destroyed savings bonds may be recovered if you accomplish the forms provided by the Treasury. Here are the steps on how to find lost savings bonds:
Step 1
Make a replacement request of unreceived or the lost savings bond(s) - If you purchased bonds but the bonds were lost even before you received it, the best thing to do is to contact the entity where you bought your savings bonds and the institutions will look at their files and if they really lost the bonds, then they can hand to you a request for replacement form. If you bought your bonds through the payroll savings plan but you did not receive it after the lapse of 15 days, contact the payroll office of your employer to verify or ask for the claim form.
Step 2
Make a replacement request of received but lost savings bond(s) - Lost, stolen or destroyed savings bonds which were already received may be replaced by the Bureau of Public Debt. You can do this if you write a claim and file it with the Public Debt Bureau upon accomplishing the 1048 form or the Claim for Lost, Stolen or Destroyed United States Savings Bonds Form. Likewise, you can download the form on the Internet off the website of the Treasury Public Debt Bureau(click on the link and scroll down a few inches to see the downloaded version of the form).
Step 3
Accomplish the 1048 form of the Treasury Department - The entries that you need to fill out consist of the description of the bonds, the details of the loss, your authority to complete the form and claim the missing bonds including the details for minors, the relief requested, the delivery address, and signatures and certifications. Don’t forget to read the instruction on the last page of the form.
Step 4
Send the accomplished Form 1048 to the following addresses - For existing HH/H savings bonds, send it to PO Box 2186 Parkersburg, WV 26106-2186 and for E/EE/I savings bonds, send it to the Bureau of the Public Debt at P.O. Box number 7012, Parkersburg, WV 26106-7012.
Step 5
